1. if you have Microsoft Excel. download this zipped file, find your chapter, fill out the form and email it to blake. 2. Sign into your oxford house email then come back here and follow the below links to use google docs. (you must be signed into your oxford house email before you follow the below links) . once you have it all filled out you can click on "File" and "email as attachment" to Blake. blake.bippes@oxfordhouse.org
